Endoscopic Sinus Surgery - ESS

Endoscopic sinus surgery (ESS) is a minimally invasive method of treating chronic sinus diseases. It involves removing inflammatory changes, polyps, or adhesions and restoring sinus patency, which improves ventilation and drainage of secretions. The operation is performed under endoscopic guidance, which allows for precise work without the need for large incisions.

Indications for ESS procedure

Chronic rhinosinusitis (CRS) not responding to pharmacological treatment
Nasal and sinus polyps causing nasal obstruction
Recurrent acute sinusitis if conservative treatment has not been effective.
Fungal sinusitis
Blocked sinus openings (e.g. cysts, adhesions)
Complications of sinusitis (e.g. abscesses, orbital inflammation)

Preparation for ESS procedure

ENT consultation and assessment of indications
Imaging tests (sinus computed tomography)
Blood tests and anesthesia consultation
Discontinuation of blood-thinning medications (e.g. aspirin, clopidogrel)

The course, advantages, and recovery after the ESS procedure

ESS operation progress

Anesthesia – most often general, less often local
Insertion of the endoscope into the nasal cavity to visualize the structures
Removal of lesions – hypertrophic mucosa, polyps, cysts or adhesions blocking the sinuses
Restoration of sinus patency – widening of sinus openings and removal of pathological changes
Nasal packing (in some cases) or use of hemostatic materials

Infectious complications (rare)

Base of skull injury with cerebrospinal fluid leak (very rare)
